Develop in collaboration with parents/caregivers individualized service plans. Continually assess needs and provide advocacy support in obtaining appropriate services. Manage community and family crises involving children and parents. Evaluate parent/child/family progress and participates in interdisciplinary team meetings. Provide guidance and support to children and their families/caregivers in a variety of environments and community settings. Document all activities in accordance with applicable regulations. Participate in case disposition meetings on a routine basis. Work independently in the family’s home and variety of community and/or treatment settings.
Qualifications: Bachelor degree in Social Work, Psychology or related field. Experience in mental health services and/or SED youth in community-based settings and/or MHRS Experience with the mental health, child welfare and/or juvenile probation systems. Knowledgeable of Department of Mental Health (DMH) paperwork, policies and procedures. Strong knowledge of case management and mental health rehabilitation principles. Must be able to work independently and exercise sound judgment in crisis situations, both in the office setting and in the field/community. Must be able to communicate thought and ideas clearly both orally and written, in both English and Spanish. Available to work evenings and weekends and able to provide rapid, real-time crisis de-escalation in the field. Must have valid CA driver’s license, reliable transportation, proof of auto liability insurance, and an acceptable driving record.
Salary: $26 - $30/hour
