The case navigator engages the child and family in the service delivery process by developing a trusting relationship.  Responsible to meet the child and family where they are within fifteen (15) business days to provide culturally competent, strength based and family-centered services, (A) accepts referrals and checks to see that the family does not have an open case with another SCSF contractor,  (B) will make telephonic contact with the referred family within 2 business days and will notify DCFS within 4 business days if unable to reach the family. Conduct strength based needs assessment with the family based in the family’s identified needs.  Provide community based linkage services for the child and family so that the family will receive the resources needed as identified in the service plan.   Follow up as often as necessary to ensure that services are identified and the child and family have connected with the resource and are receiving services as planned.  Continually assess needs and provide advocacy support in obtaining appropriate services.  Manage community and family crises involving children and parents.

Qualifications: Bachelor degree in Social Work, Psychology or related field.  Experience in mental health services and/or SED youth in community-based settings and/or case manager experience with the mental health, child welfare and/or juvenile probation systems.  Must be able to work independently and exercise sound judgment in crisis situations, both in the office setting and in the field/community.  Must be able to communicate thought and ideas clearly both orally and written.  Ability to represent core/agency/program values and principles in a variety of settings.  Bilingual English/Spanish required.   Must have reliable transportation, valid CA driver’s license, acceptable driving records and proof of current auto insurance.

Salary: $21 - $24/hour
